Alabama company turns gun lovers' ashes into ammunition (Reuters)

MOBILE, Ala (Reuters) ? There's something to be said for going out with a bang.

Two Alabama game wardens have devised a smoking send-off for avid hunters and gun enthusiasts: For a small fee, the pair will turn cremated ashes into ammunition that the deceased's loved ones can fire at will.

Holy Smoke LLC has serviced only two clients since launching in July, but founders Thad Holmes and Clem Parnell said they have seen an uptick in prearrangements thanks to word-of-mouth and a recent flurry of international press.

"It's about celebrating life," Holmes, a 16-year state conservation officer, said on Friday. "We know how strange it sounds to people who aren't comfortable around guns, but for those who are, it's not weird at all."

Four years in the making, Holy Smoke was born during a late-night stakeout to thwart illegal hunting. Parnell had recently lost a family member, and the conversation shifted to their own reservations about traditional methods for disposing of the departed.

Parnell and Holmes dreamed up a company that would fill shotgun shells and rifle or pistol cartridges with ashes, allowing gun enthusiasts to spend eternity the way they lived their lives.

"People take ashes and spread them across lakes or forests or throw them in rivers, and nobody thinks twice about that. This is no different," said Holmes, who noted that a pound of ash fills about 250 shotgun shells.

Costs start at $850. In addition to handling the ashes of one client each from Kentucky and Florida so far, the Stockton, Alabama, company has received inquiries from members of law enforcement, fire departments and the military who wish to have their service honored in that way when the time comes.

Other clients have requested the service be carried out for their pets, particularly bird dogs who have remained faithful companions throughout the years, Holmes said.

Joyce Harrison of Spanish Fort, Alabama, said she understood why some people might recoil at the concept. But she wishes the option had been available seven years ago when her husband of 42 years died of cancer.

"Toward the end he wasn't able to spend as much time hunting as he liked, but he took off any time he felt strong enough to get through, and he always came home so relaxed and at peace with everything," Harrison said. "This would have been perfect for him."

Ivory Coast launches reconciliation process

(AP) ? Politicians and Ivory Coast's most famous soccer player are launching a Truth and Reconciliation Commission after months of postelection violence.

At least 3,000 people were killed from December to April, when United Nations airstrikes finally forced the country's entrenched ruler Laurent Gbagbo to cede power to the winner of the poll Alassane Ouattara.

The South African-style Truth and Reconciliation Commission being inaugurated Wednesday is expected to hear grievances from the families of people killed by Gbagbo's military.

It also will hear testimony about those murdered by the rebel fighters Ouattara enlisted to help him take power. Chelsea soccer star Didier Drogba is expected to be one of the members of the commission.

Technique gives precise picture of how regulatory RNA controls gene activity

Thursday, September 29, 2011

A new technique developed by researchers at the Stanford University School of Medicine allows researchers to identify the exact DNA sequences and locations bound by regulatory RNAs. This information is necessary to understand how the recently identified RNA molecules control the expression of neighboring and distant genes.

The study offers a startling glimpse into the intricate world of gene expression and how RNA, once thought to be only a lowly cellular messenger, actively unlocks our DNA-based genome. "We used to have to just infer where these RNAs were acting based on their biological effects," said Howard Chang, MD, PhD, professor of dermatology. "But now we can identify precisely where on the chromatin they are binding. We've found that these sites are focal, numerous and sequence-specific."

Deciphering the role of regulatory RNAs is critically important to understand many cellular functions, including those involved in development, cancer and regeneration.

Chang, who is also a member of the Stanford Cancer Institute, is the senior author of the work, which will be published Sept. 29 in Molecular Cell. He and his lab previously identified some of the first known regulatory RNAs, including one in 2007 they called HOTAIR. Graduate student Ci Chu is the first author of the research.

Fifty years ago, researchers first identified messenger RNAs, which serve to transcribe the genetic instructions encoded in the DNA from the nucleus and deliver it to the cell's protein-building machinery. Over time, the unidirectional flow of information from DNA to RNA to protein became known as biology's "central dogma." Regulatory RNAs challenge that notion by binding to DNA and affecting which genes are selected to become proteins.

The existence of these regulatory RNAs, also called long intergenic non-coding RNAs, or lincRNAs, was first postulated decades ago by researchers who realized that DNA packaged into chromatin (a coiled complex of DNA and proteins) contains more than twice as much RNA by weight as DNA. They wondered if the RNA coating the chromatin actively regulates when and how specific genes are turned on and off. But technical difficulties have made it difficult to identify precise sites of binding until now.

"To capture this picture, you have to trap that interaction between lincRNAs and chromatin in living cells," said Chang. "It was not at all obvious how to do that."

In the end, Chu devised an innovative riff on a common assay used to identify the DNA sequences bound by a class of proteins called transcription factors. For that experiment, researchers mix protein and chromatin and then use antibodies to isolate, or immunoprecipitate, both the protein of interest and its preferred binding site.

Instead of using an antibody, Chu and Chang used two or three chemically tagged, complementary nucleotide sequences, or probes, to isolate the regulatory RNAs after they had bound to the DNA. However, they could capture only about 10 percent of the regulatory RNA using this method. Eventually Chu hit on the idea of using dozens of individually labeled nucleotide sequences that bind throughout the molecule.

"The problem was that RNA is a long, floppy molecule," said Chang. "During the experiment, it would get broken into pieces and the chemical tags would be lost. So, since we didn't know exactly which portion of the lincRNA binds to the chromatin, we made a series of tagged probes to bind to every part of the lincRNA." This "tiling" approach ensures that, even if the RNA molecule is fragmented during the procedure, the researchers will still be able to isolate the small portion that remains bound to chromatin.

Using the new technique, Chu and Chang were able to investigate the binding specificity of three lincRNAs ? one from the fruit fly, and two from mammalian systems. They found that one, called roX2, seems to coat genes on the single X chromosome in males and facilitate gene expression.

"We showed that roX2 very precisely binds to the X chromosome only in male cells," said Chang, "and it does this in a gradient, binding more and more at the transcriptional end of the gene. This appears to help elongation and make transcription more efficient, which is important because male animals have only one copy of the X chromosome as compared to a female's two."

Another lincRNA from humans, a component of the telomerase complex called TERC, occupies not just a chromosome's telomeres (protective caps at the end of chromosomes), but also genes involved in the Wnt pathway, which has been shown to control self-renewal.

"This shows an interesting and direct connection between the machinery for chromosomal replication and self-renewal," said Chang. "It's possible this helps the cell monitor the status of its telomeres."

Finally, HOTAIR, first identified in Chang's lab, seems to recruit a gene-silencing protein complex called polycomb to bind broad swaths of DNA.

"Taken together, our research shows that our technique is widely applicable and can vastly enrich our understanding of how regulatory RNAs unlock the genome in many very specific ways," said Chang. "It's very exciting."

Elderly patients may be undertreated for prostate cancer, study suggests

ScienceDaily (Sep. 26, 2011) ? It's an ongoing debate: Should men over a certain age be treated for prostate cancer? Should these patients be submitted to treatments that may result in significant side effects if they may not live very much longer?

Now, a study by researchers at UCLA's Jonsson Comprehensive Cancer Center has shown that men over 75 with prostate cancer are being undertreated, while patients with a single comorbid condition such as peripheral vascular disease or those in wheelchairs are being over-treated and doing much more poorly than expected.

"This study suggests that men with certain medical conditions are being treated just as aggressively for their prostate cancer as men who are healthy, even though they are more likely to die of non-prostate cancer causes. Conversely, men over 75 are not being aggressively treated when they may, in fact, fare better than younger men with complicating health issues," said Dr. Karim Chamie, a postdoctoral fellow in urologic oncology and health services research and first author of the study. "There are individual medical problems that impact survival much more so than simply being 75 years of age."

The retrospective study, which looked at the records for 1,031 men diagnosed with localized prostate cancer between 1997 and 2004, appears in the early online edition of the Journal of General Internal Medicine.

The patients studied were being treated at the Greater Los Angeles and Long Beach Veteran's Affairs Medical Center, where care is subsidized through a federal program. So there was no financial incentive to treat patients in the study, removing physician reimbursement from the equation.

"Because these are VA patients, it eliminates any speculation or concern that treatment decisions were being made with reimbursement or the bottom line in mind," Chamie said. "This really speaks to the underlying psychology of physicians and how they manage patients with prostate cancer."

In this study, Chamie and his team sought to determine the probability of prostate cancer treatment and survival among men with a single comorbid condition. They measured the risk of undergoing treatment for men with a host of medical problems, such as chronic obstructive pulmonary disease (COPD), diabetes with and without organ damage, congestive heart failure, stroke, heart attack, liver disease, peripheral vascular disease and other comorbid conditions.

Even though prostate cancer patients with the comorbid conditions had lower survival rates than those without, they were being treated just as aggressively. Approximately two-thirds of men with or without comorbidities underwent aggressive treatment for their prostate cancer.

The study found that only 16% of men with prostate cancer who had no comorbid conditions died of other causes at the 10-year mark. Conversely, in diabetics with no associated organ damage, 35% were dead after 10 years of causes other than their cancer. In men with peripheral vascular disease treated for their cancer, 49% were dead at 10 years. In diabetics with organ damage, such as kidney or eye problems, 50% were dead at 10 years out. And in men with moderate to severe COPD, 65% were dead at the 10-year mark.

The study also examined the age of patients and found that only about a quarter of men aged 75 were being treated for their prostate cancer, even though their chances of living another 10 years were 58%.

"I think some physicians may think that a man with only one comorbid condition is healthier than he may in fact be," Chamie said. "Some patients, even those with just a single comorbid condition, might benefit from surveillance, as they may die as a result of other causes before their prostate cancer and avoid being exposed to some of the debilitating side effects of treatments."

"This study suggests that while physicians use patient age of 75 as a very strong cue to avoid overtreatment of men with prostate cancer, they are less likely to integrate certain comorbidities into prostate cancer decision making, "said Dr. Mark S. Litwin, professor and chairman of the urology department and a researcher at UCLA's Jonsson Comprehensive Cancer Center. "There are some medical conditions that portend a poor prognosis and should therefore be taken into consideration in the decision-making process."

The study was funded by the American Cancer Society, the Ruth L. Kirschstein National Research Service Award, a Jonsson Comprehensive Cancer Center Seed Grant and the National Institute of Diabetes and Digestive and Kidney Diseases.

Baseball Safety Rules That Should be Followed | LibPress.com

While there?s always the risk of injury when playing baseball, or any sport, by taking certain safety precautions, players can substantially reduce the risk of problems on the field. By staying in shape, wearing the correct gear and focusing during the games and practices, the chance of getting injured decreases. Let?s talk about a few of the main things that you should remember if you want to continue being safe when playing baseball.

Sometimes players can be much safer simply by talking to each other at crucial times. When infielders or outfielders are trying to catch a ball, for instance, they have to communicate with each other. It?s standard practice for fielder to call out, ?I?ve got it!? or something similar, and this is essential to avoid collisions. The player who calls for it first should be given the room to go after it, though rarely two players may call for it at the same time.

Coaches should prepare players for this type of situation by conducting drills and instructing the players who has priority over which sections of the field. Both infielders and outfielders must learn to keep an eye on other players as well as the ball for safety reasons.

A danger in the game of baseball that we don?t always realize is the bases. When a player slides hard into a stationary base, it?s easy to injure the foot or leg. If the player is sliding headfirst this can cause head trauma or injury to the arm or hand. Breakaway bases offer a solution to this danger because they offer the ability to ?break? away when the player slides into them. These bases are stable during normal play. A simple suggestion might get your league on board with this safety feature if they aren?t already. Aside from this, players should practice proper sliding techniques and be aware that bases can be dangerous.

It is imperative for baseball players to keep themselves well hydrated during play especially in hot weather. It?s important to watch players especially the young ones to ensure they are drinking enough to stay hydrated especially during game play. Watch out for heat exhaustion and sunstroke in warmer weather climates even for the most healthy players. Calling off games due to heat is a good idea if it?s too extreme. Parents of young players need to use their best judgment when letting their child play in hot temperatures. It?s critical for your players to be well hydrated no matter what the weather. Baseball is a sport with timeless appeal. By abiding by these safety guidelines in this article, players can participate in the game with less chance of injuries. You do not have the power to predict when things will go wrong. If players are properly attired and following sensible precautions, the game can be made much safer.

Good Moms Seem to Help Poor Kids Become Healthy Adults (HealthDay)

SATURDAY, Sept. 24 (HealthDay News) -- Poor children are more likely to become unhealthy adults -- vulnerable to infection and disease -- than kids from higher-income families, according to a new study.

However, the study findings revealed, some disadvantaged children grow up into healthy adults. Their secret: a nurturing and attentive mother.

Upward mobility also has been cited as a reason that children from low-income families become healthy adults, the study pointed out. Yet the researchers found that income in adulthood didn't offset childhood poverty.

"But those greater risks later in life seem to be offset if the mom paid careful attention to the children's emotional well-being, had time for them and showed affection and caring," Gregory Miller, lead study author and psychologist at the University of British Columbia in Canada, said in a news release from the Association for Psychological Science.

For the study, clinicians performed physical examinations on roughly 1,200 adults and researchers rated their socioeconomic status based on level of education. The investigators also surveyed the participants to determine how well their parents nurtured them as children.

The study, scheduled for publication in an upcoming issue of Psychological Science, found that the more wealth a child's family had, the better the individual's health was in adulthood.

Children whose parents did not graduate from high school were 1.4 times more likely to develop the metabolic syndrome (a set of risk factors linked to heart disease, diabetes and other health problems) than those raised by college graduates. But children with nurturing mothers were the exception.

The authors suggest that reducing and managing stress throughout childhood and later in life is what makes the difference.

"There is a lot of evidence that nurturant moms" -- or possibly other adults -- "can help buffer vulnerable kids from all sorts of negative outcomes," Miller stated in the news release.

Previous research has found that stress endured by disadvantaged children permanently affects their physiological development, making them more vulnerable to disease, the study authors noted in the news release. These children are more likely to have a cluster of symptoms -- including high blood pressure and abdominal fat -- that put them at risk for the metabolic syndrome.

Miller concluded that parents can help their children become healthy adults by teaching them how to cope with stress effectively and by being a good example of appropriate emotional responses.

"We can do lots to help kids get through tough times," Miller said.

Step # 3: Inspect the house or property with caution

In addition to determining the market, you must also personally verify the home, apartment or property you want to buy. Even if your broker says that the house is in good condition, you still have to take some time to inspect the property before you ask for a mortgage because your standards can not be the same as your agent.

You should check the area and determine whether all public services work, and leaks and other things you need to make the house habitable.

Step 4: Find the best mortgage

If this is your first home, you probably need a mortgage to buy property. Do not accept the first mortgage offer that you can get. It is worthwhile to shop around for bargains. Remember that even 0.05% of the difference between the interest rate can be a big problem, especially in difficult times.

Development Finance Vs. Commercial Mortgages | Loan ...

There can be a lot of confusion surrounding Development Finance and what it implicates within the financial world. A question that is frequently asked is: What is the difference between Development Finance and Commercial Mortgages? And this is where the confusion often stems. Development finance is where an individual or company/business is looking to develop property/properties and have some capital but need a short term loan to help complete the development. Depending upon the lender and the circumstance, such loans normally span between 12-24 months. Commercial Mortgages on the other hand, are usually only required once the development has been completed and additional funds are necessary. Hence, Development Finance and Commercial Mortgages do tend to overlap. Despite the "Credit Crunch", Development Finance is rapidly becoming more main-stream and is a very specific type of finance. High street lenders are very active within this market as development finance is popular with people, small and large businesses alike. It is recommended that you seek professional advice in order to find the right deal for you.

Development Finance is most popularly used in the UK for such projects as Property Refurbishment, Property Conversions and New Build Projects.? Additionally, there are various types of Development Finance which undoubtedly adds to the confusion and uncertainty surrounding the term.?? For example, a Senior Debt Loan is the most popular as it covers the first 70%-80% of loan to value and a Mezzanine Loan is second to the above form and helps the developer if his money is tied up elsewhere in different things. It is to be noted that this is by far a definitive list.

?Property development is about having a vision; it's about understanding the market and turning that vision into a reality. However, developers often have problems getting the finance right and knowing what products are available and which lenders to use can be confusing. Finding the right form of Development Finance for you depends entirely upon your financial needs, whether you are a homeowner wishing to refurbish, a business looking to expand or an individual with a vision to start anew. Community projects also use this form of finance in supporting people and businesses in disadvantaged communities, both in the UK and worldwide. Therefore, Development Finance is determined entirely upon an individual assessment made by the lender. All aspects of the development proposal have to be faultless in the eyes of the lender before they agree to provide funding. Lenders look meticulously at development characteristics such as land purchase, ground work, labour and services, first and second fix and then sign off. In the difficult current market, lenders have to be more careful when choosing which developers to back; they are much more likely to support a developer with experience in the field than someone new to the industry.

?Development Finance lenders are there to build a relationship with the developer in order to share their vision and provide the support needed to make that vision a reality. The loan can cover costs such as building costs, labour and can often include any architect costs and other professional fee's such as wages and drawings. Property development loans will be secured against the land or the property you wish to develop. Newer forms of Development Finance funds are available either for debt, mezzanines or equity in combination with primary lending sources. More traditional forms, like with Commercial Mortgages, normally require a deposit of at least 20-30%.

?Loan to Value rates and interest rates vary depending upon experience and percentage of funds required for development. Advantages of this form of financial assistance include it can be raised quickly, turning your dream into reality sooner rather than later and that each proposal is considered on its individual merit. Another advantage of this form of finance is that the lender will always be there to help manage the development programme and lend support. So, whether you intend to use your finished project as an investment or whether it's used to help your business grow, Development finance is a fully supportive and flexible form of financial assistance to help you make that vision a reality. As with any financial decision research the market thoughrouly before proceeding, look for the best commercial mortgage rates available and be aware that there may be other commercial property finance options available.

Which Factors Influence Foreign Exchange Rates?

Foreign currency exchange (widely known as Forex or Fx) immensely spread financial market operated all across the world. An individual to large scale organizations, banks and even governments are involved in forex trading. The fundament of forex trading depicts that the currency is bought when its value is low and it is sold immediately when the currency value gets strengthened. Today Forex is the highest profit oriented market incurring trillions of cash turnover every day. It is the only market that stands top in terms of liquidity.

Many of us may not know how the market operates yet we have an idea that trading is based on fluctuation of foreign exchange rates. Basically the foreign currency rates are the currency values of various countries globally that keeps rapidly fluctuating. Hence it is very significant to keep eye on the changing market trends and the foreign currency rates for everyone who is involved or need to perform an international transaction.

    Significance of foreign exchange rates
Every person have one objective whether he is trading into Fx or need to perform an occasional international transaction, that objective is to save money or make profit. This involves buying or exchanging any currencies when the buying currency rate is low. Currency rate does change on various factors and global events that are unpredictable in nature. To better explain the effects of fluctuation of foreign currency rates I am giving below example:

For e.g. Current currency exchange rate of 1 USD = 0.61 GBP. Now I want to purchase jewelry worth 6000 GBP from the UK to my home in the USA. If I buy as per current rate I will be paying around 10,000 USD towards my purchase. But if I postpone my decision to tomorrow the currency rate can get a fluctuation to may be 1 USD = 0.56 GBP, which will require me to pay few hundred dollars more. And the situation can be vice a versa. Hence an individual can make profit or lose is unpredictable when it comes to foreign currency rates exchange.

Which are the factors responsible in influencing foreign exchange rates?
It works like a demand and supply of a commodity. For eg. Fruits are been exported from Europe to various countries. Due climatic changes the stock got destroyed which will lead to a scarcity in the market. Ultimately the price of the fruit will be raised because of low supply. Similarly there is demand and supply rule applicable in currency rate fluctuation too. The demand of a particular currency in the market can result in the currency rate change. Investors will always demand the currency that can give high return on investment and this way the demand and price of the currency gets reflected.

Foreign exchange rates fluctuates due to various factors like economic condition of the country, International trade policy, Government, national income, bank savings, inflation etc. can influence variations in foreign currency rates. Disturbance caused in demand and supply of country?s basic merchandise/goods due the natural factors like flood, earth quake, etc. can also impact on the country?s currency rate.

Investors fear governments can't stop recession

By John W. Schoen, Senior Producer

Global stock markets are falling in lockstep this week because investors all share a common fear: The world is slipping back into recession and governments around the world are powerless to stop it.

Two years after the world emerged from the worst financial collapse since the 1930s, a repeat is looming. European Union officials have been unable to forge a political consensus among its 17 members to head off a debt default by Greece. Some of Europe's banks won't have the capital to withstand the expected losses on their holdings of Greek debt.

European leaders are now preparing for an "orderly" default, according to published reports.

Though they've cobbled together a fund to backstop Greece's $500 billion debt, it's not enough to cope with a possible default four times as big by Italy, which is in the early stages of the downward spiral that sank Greece.

"Greece is gone, the markets know that," Allan Sinai, chief economist at?Decision Economics. "We're now looking at Italy and Spain and the potential for Italy going. And that's what's so scary."

Scarier still is the growing sense that Europe's political leaders are powerless to stop the downward spiral. Despite repeated assurance that they are prepared to act, the European officials have been paralyzed by a deep political division over wealthier countries like Germany and France should bail out their weaker neighbors. As the crisis has deepened, the political divide has widened.

Leaders of the 20 largest economies, meeting in Washington this week, made yet another pledge to act.

"We are taking strong actions to maintain financial stability, restore confidence and support growth," the G-20 joint statement said. "We commit to take all actions to preserve the stability of banking systems and financial markets as required."

But there was little indication of any action to back up the statement.

The stock market appeared to find a bottom Friday after a week of heavy selling that wipe out?roughly?$1 trillion in market value as measured by?the?Wilshire 5000, one of?the?broadest market?indices. ?

In recent weeks, the strongest action has come from the U.S. Federal Reserve, which announced this week an historic, and highly unorthodox, program of reshuffling $400 billion worth of its bond holdings to try to push interest rates even lower.

But, because the plan is widely expected to do little to revive the stalled U.S. economy, the move only served to remind investors that the central bank has run out of effective options.

"The fear that it's all dependent on the Fed -- together with this mess in Europe -- is really getting people more and more worried, unfortunately," Jim O'Neill, chairman Goldman Sachs Asset Management, told CNBC. "And there's just no sign of any true leadership in Europe."

The problems that those leaders now face would be daunting even if a political consensus could be reached. Massive debts taken on be Europe's weaker countries have slowed growth, raising investor fears that those debts may be unsustainable. Those investor fears have forced interest rates higher, raising borrowing costs, forcing deep budget cuts, which accelerate the economic contraction. The downward spiral is now feeding on itself.

"If you take the debt of the periphery countries, combine them with the bank debt because they're joined at the hip, you come up with a number of about 5.5 to 6 trillion euros," Sean Egan, an analyst following the debt crisis at Egan-Jones Ratings Company, told CNBC.

Even if only a third of that debt goes bad, said Egan, it's more that the stronger European countries can manage.

"Germany, which is held up normally as a savior, has total debt in the area of 1.5 trillion euros ($2 trillion)," he said. "It's unlikely they're going to be willing to double their debt to bail out the periphery countries. That is the basic problem."

In the U.S., Congress and?the?White?House?remain deeply?divided?over how to close a $1.4 trillion budget deficit, having succeeded in only?in postponing the day of reckoning until year-end. That process will only become more difficult as a deeply partisan presidential?campaign intensifies the debate.?

Compared to their European counterparts, U.S. banks are relatively well capitalized to withstand another financial crisis. And, despite recent attacks by Republican Congressmen and president candidates, the Federal Reserve remains a lender of last resort to U.S., ready to backstop the U.S. financial system if another meltdown were to happen.

American companies, having paid down debts and raised mountains of cash since the Panic of 2008, are also better positioned to weather another financial shock.

But with a housing market mired in a four-year recession, unemployment at levels not seen in decades and governments around the world slashing spending, the global economy would enter a new recession in much worse shape than the downturn of 2007.

That recognition has brought an abrupt change in investor thinking about how well companies will fare if they're entering a much more difficult economic environment.

"Corporations were generally be happy about life in late August early September '08 as well," said O'Neill. When the financial system really imploded and financial firms stopped extending credit to anybody, the corporate world had to destock and we know what happened after that. We are not far off the same sort of thing."?

Mamas on Bedrest: Pregnancy Spacing Improves Long Term Health ...

Mamas on Bedrest: Pregnancy Spacing Improves Long Term Health Of Mamas and Babies

    September 23rd, 2011

According to a February 1999 New England Journal of Medicine article, separating pregnancies by 18-23 months is optimum to ensure the health of both mamas and babies.

The authors,? Bao-Ping Zhu and colleagues found that incidences of adverse pregnancy outcomes such as premature birth and low birth weight were less likely to occur in pregnancies in which conception occurred 18-23 months after a prior pregnancy and delivery. They also found that there were lower incidences of pregnancy complications, such as pre-eclampsia, in pregnancies which occurred 18-23 months after a previous pregnancy and delivery. Interestingly, rates of complications went up when the pregnancy intervals were longer than 23 months between pregnancy and subsequent conception. The authors admitted that while this was a first assessment, there may be likely confounding variables such as maternal age, socioeconomic status, reproductive history and others that may exert an influence on pregnancy outcome.

As you can imagine, this has not always been well received. Many people simply balk at the notion of ?planning? pregnancies and feel like pregnancies occur when they are supposed to occur. Others contend that the failures of contraception account for most of the pregnancy failures and that this cannot be helped as not contraceptive method, except for abstinence, is 100% effective at preventing pregnancy.

At the 2011 Association of Reproductive Health Professions Annual Meeting, this topic was discussed and the position of the Reproductive Health professionals present is to recommend long acting contraception (LARC) to women, especially those in the highest risk groups. Robert Hatcher, MD, MPH reviewed the currently available forms of reversible contraception.

    1. Depro Provera Injection
    2. Paragard IUD
    3. Mirena IUS
    4. Implanon implants

The recommendation for LARC comes as unintended pregnancies in the United States account for approximately half of all pregnancies annually. 60% of unintended pregnancies are what is called ?Mistimed Pregnancies? meaning women admit that they would have had (another) child, just not at the time of this particular pregnancy. Most women cited contraceptive failure as the primary reason for unintended pregnancy. But a closer look at contraceptive use habits revealed some interesting statistics.

James Trussell, PhD, Office of Population Research at Princeton University and The Hull York Medical School shared statistics that showed that while the rates of unintended pregnancy has dropped amongst teens, the rates have increased in women in their 20?s. Women of lower education and lower socioeconomic status account for the greatest numbers of unintended pregnancies and African American and Latina Women have the highest rates of unintended pregnancies.

What was even more alarming is that in 2001, 52% of unintended pregnancies were to women who were using no method of birth control. Further, when interviewed for a study between 2006 and 2008, 10.6% of women at risk for unintended pregnancy weren?t using any contraceptive method.

Contraceptive problems arise mostly from ?typical use?. What this means is how women typically use the contraceptive method vs. ?Perfect use? i.e. how the method is intended to be used. Below is a table taken from the 2011 Contraceptive Technology Handbook outlining the failure rates of contraceptives with ?typical use? and ?perfect use?.

    Method????? Typical Use????? Perfect Use

    Chance 85% ? ? ? ? ? ? ? ? ? 85%?? (Percentages are effective rates)

    Condom???????????????????? 18%????????????????????? 2%

    Pill, Patch, Ring??? ? ? 9%???????????????????? 0.3%

    Depo Provera??????????? 6%???????????????????? 0.2%

    Paragard IUD?????? ? 0.8%??????????????? ?? 0.6%

    Mirena IUS ?????????? 0.2%????????????????? ? 0.2%

    Implanon?????????????? 0.05%??????????????? 0.05%

What this table shows is that methods that require consistent (daily) use have a significant failure rate and significant difference between ?typical use? and ?perfect use?. However, the more ?reliable? methods provide no protection against sexually transmitted infections (STI?s).

Anita Nelson, MD, Professor at the David Geffen School of Medicine at Harbor-UCLA Medical Center in Manhattan Beach, CA looked specifically at the oral contraceptives or birth control pills. What she shared both from the research and from her years in clinical practice is that in a 12 month cycle, women rarely take their pills as prescribed. Women on average miss 3 pills a month and as many as 60 pills a year. Even if they take a pill when they remember, for many women, this is days later and they are already at risk for unintended pregnancy.

Nelson also reiterated a little known fact amongst many women. Pregnancy is more dangerous to a woman?s health than hypertension, blood clots in the legs or diabetes and yet pregnancy increases the risk of all of these conditions occurring and persisting throughout a woman?s life.? Pregnancy related mortality (death) in the United States between 1998 and 2005 has been higher than at any other time in the previous 20 years. 14.5 women die annually for every 100, 00o births and the rates for African American women is 3-4 times higher. Unintended pregnancy also has a higher risk of ?sicker babies?.? Nelson and other researchers advocate continuous (or long acting) oral contraceptives and condoms as a way for,

    • Women to control their fertility
    • Pregnancy to occur when desired, lowering risk for complications
    • Unwanted fertility to be eliminated

Family Planning is seen by the US Centers for Disease Control and Prevention as one of the top 10 most important contributions to public health in the 20th century.

Family Planning is also seen as an important global health issue asserts Willard Cates, Jr., MD, MPH of Family Health International and The UNC Gillings School of Global Public Health. Cates presented data and information from the United Nations Population Fund, an international development agency that promotes the right of every woman, man and child to enjoy a life of health and equal opportunity. Sharing statistics from the Guttmacher Institute, family planning averts 187 million unintended pregnancies and in turn prevents:
    ? 54 million unplanned births
    ? 112 million induced abortions
    ? 1.2 million infant deaths
    ? 230,000 maternal deaths
    ? 71 million DALYs saved

(WHO Definition of DALY?s: DALYs = Disability Adjusted Life Years. The sum of years of potential life lost due to prematuremortality and the years of productive life lost due to disability)

Cates reiterated that Family Planning contributes to the Millinium Development Goals for the world which are:

    1. End Poverty and Hunger
    2. Universal Education
    3. Gender Equality
    4. Child Health
    5. Maternal Health
    6. Combat HIV/AIDS
    7. Environmental Sustainability
    8. Global Partnerships

Cates makes the case that only with widely available, long acting reversible family planning will the world?s goals of economic equality for women, increased educational opportunities for women, improved health and mortality for women and babies, reduced unintended pregnancy rates, reduced abortion rates, increased economic growth and stability for all nations.

What are your thoughts on contraception and unintended pregnancy? Would you ever use a long acting reversible contraceptive (LARC)? Share your comments below.
